MDS Coordinator

The MDS Coordinator for Faith Lutheran Home is responsible for assuring the function of the interdisciplinary team. The position is responsible for assuring completion of each resident’s MD and Care Plan. This includes assessment of the resident’s health care needs and communication and implementation of the plan of care to direct care staff. 

Qualifications: Applicants must possess a registered nurse degree through an accredited program and a current Montana RN License. Additionally, two years of experience in nursing with at least one year of experience in long-term care and one year of successful nursing supervisory experience or completion of a course in management principles.

RN - Discharge Planner / Case Manager - Trinity Hospital

The Discharge Planner / Case Manager, RN conducts admission, concurrent with retrospective utilization reviews for all assigned patients. This individual communicates with payers, physicians and the VP of Nursing Services, to facilitate appropriate admissions and discharges, and to promote a high quality of care while preventing under-utilization and over-utilization of hospital resources. This person conducts discharge-planning activities for assigned patients and completes data collection This person also is responsible for an active utilization review of all patient admissions and discharges, collaborates and facilitates on all Swing Bed admissions, maintains an active re-admission review program, and works closely with the VP of Nursing Services and the Nursing Leadership team to improve patient outcomes.

Qualifications are: Possess a Montana Register Nurse degree through accredited program. Current Montana R.N. license

2 years acute care experience, Training in patient assessment and identification of patient needs, a working knowledge of Microsoft Office, Excel, PowerPoint and a Current BLS or CPR certification (required), ACLS, PALS, NRP, and TNCC and as indicated by work area, and Nonviolent Crisis Intervention (required within two years of hire).

Surgical RN Circulator  - Trinity Campus

Hours: Full Time or Part Time, 2-4 days a week, day shift. On-call required.

Pay: Negotiable depending on experience. A bonus is offered for call pay during the weekends and the hourly call pay is higher for weekends and holidays.

Requirements: Must possess an RN degree through an accredited nursing program and must be licensed by the State of Montana Board of Nursing. 1+ years of general nursing experience preferred. Job Summary: The RN Circulator provides patient-specific care to patients during the pre-operative, intra-operative, and post-operative phases within this specialty area of nursing. The RN Circulator is accountable for the coordination of care of the patients going into the OR (Operating Room), PACU (post-anesthesia care unit or recovery room), and GI Lab (Endoscopy). Job Summary: The Surgical Technologist (ST) performs duties within the perioperative phases of care. The ST will perform a sterile scrub prior to surgery, set up instruments using proper sterile technique, and pass instruments to the surgical team during surgeries. The ST will be knowledgeable regarding the proper transport of instruments as well as ensuring that all surgical instruments, supplies, and equipment are properly decontaminated, inspected, sterilized, and stored after the surgery is completed. The ST will assist with non-surgical procedures, within their scope of practice, which includes procedures in the GI Lab. The ST will perform other duties as assigned while maintaining professional communication with members of the surgical team.
